Lexington Assisted Living and Memory Care is looking for a positive, proactive clinical professional to lead out the wellness team of caregivers and medtechs. We are looking for a proven leader who can inspire, train and hold a team accountable. Ideal candidate would be a business partner to the Executive Director, have an ownership mindset and want a challenging and rewarding opportunity. Must have strong time management skills and can prioritize complex situations in a rapidly moving environment. Must be approachable and communicate effectively and timely for our families, residents and staff as well as outside providers.

Primary Duties may include:
-Lead and Manage the care team including the Wellness Coordinator
- Coordinate care for residents including assessing residents and writing individual service plans and updating EMAR/PCC. Maintain accurate and up to date medical records.
-Ensure we are compliant with state regulations
- Educate patients and care staff on wellness practices, disease prevention and infection control.

- Valid nursing license in the state of California
- Proven experience in a medical office or healthcare setting
- Strong knowledge of anatomy and medical terminology
- Excellent communication skills, both verbal and written
- Attention to detail and ability to multitask in a fast-paced environment
- Strong organizational skills and ability to prioritize tasks effectively
