The Legacy Senior Communities is Hiring an Occupational Therapist (PRN) Near Dallas, TX

The Occupational Therapist (PRN) will plan, organize, implement, and evaluate a progressive therapy program geared to improvement of member’s, resident’s or patient’s physical functions, psychological, social, and economic wellbeing. Supervise Certified Therapy Assistants and rehab assistants. Follow community guidelines, objectives, and policies to maintain standards of treatment consistent with legal and accrediting requirements.
  • Upon receiving OT orders, review member, resident or patient’s history and completes thorough assessment, including precautions.
  • Upon receiving OT screening request, review member, resident or patient’s current situation and screens
  • individual for intervention.
  • Communicate assessment/ screening results to member, resident or patient and appropriate family and/or staff.
  • Complete required assessment/screening documentation.
  • Identify and prioritize needs based on assessment and establish appropriate plans with goals.
  • Communicate treatment plan and goals to members, resident or patient and family.
  • Collaborate with health care team members to begin planning for appropriate and timely discharge or transfer.
  • Demonstrate ability to prioritize treatment procedures according to member, resident or patient’s needs and MD requests.
  • Implement interventions according to plan of care.
  • Communicate purpose of treatment procedures to member, resident, or patient/family according to level of understanding and modify plan accordingly.
  • Communicate treatment plan/appropriate interventions with assistants, initially and ongoing.
  • Communicate ongoing treatment issues/plan to interdisciplinary health care team members.
  • Complete required documentation regarding daily intervention.
  • Review and evaluate effectiveness of interventions toward achieving patient outcomes
  • Document appropriate reassessment and changes in plan of care.
  • Communicate significant changes in intervention to member, resident or patient /family and health care team members.
  • Complete required documentation for discharge.
OTHER DUTIES:
  • Perform other related duties as assigned
  • Support and participate in changes and suggestions made by Director of Rehab
GENERAL FUNCTIONS:
  • Maintain confidentiality of resident, employee, and facility information.
  • Follow Resident’s Rights policies at all times maintaining privacy, confidentiality, and respect.
  • Ability to understand and comply with applicable Federal, State, and local regulations as they apply to this job.
  • Attend all required department and/or facility meetings.
  • Adhere to applicable laws, rules, regulations, and Code of Conduct directed by Corporate Compliance Program, conduct business using ethical standards, and report any suspect or actual event or circumstance to the Compliance Officer.
  • Understand and comply with use and disclosure rules of resident Protected Health Information (PHI) as required by the HIPAA Privacy Rule of the Health Insurance Portability and Accountability Act.
  • Use the proper equipment for each job and report any missing or damaged equipment to supervisor immediately.
  • Follow facility safety program, identify, and report unsafe equipment, environment, or procedures to supervisor.
  • Read and follow instructions on Safety Data Sheets (SDS) when handling or using chemicals.
  • Assume accountability for data contained in the employee handbook.
  • Observe all facility policies and procedures.
  • Complete employee training courses as assigned by due date.
  • Perform any other related duties as directed by supervisor.
Educational Requirements & Experience
  • Must have current physical examination showing freedom from certain types of communicable diseases.
  • Must be a graduate of an accredited Occupational Therapy program with a BS degree.
  • Must be approved and hold a valid certification by the NBCOT and state of Texas.
  • Must have fieldwork experience in geriatrics or hospital acute/sub-acute/rehab.
  • Must have current Cardio Pulmonary Recitation Card (CPR) issued by recognized provider such as American Heart Association or American Red Cross that includes hands-on practice and in-person skills assessment or be able to obtain CPR training and card within 30-days of hire date.
