The Leaf Company/ Parents in Training is Hiring a Trauma Support Specialist Near Chico, CA

Job Summary:

The Trauma Support Specialist will provide therapeutic guidance to the Parent Education and Support Coordinators who work with families, coaches, respite providers, and supportive services. The Trauma Support Specialist is responsible for providing direct therapeutic support and direction to the Parent Education and Support Coordinators in their roles as identified in their job description.

The Trauma Support Specialist will provide monthly family visits to gauge parental readiness to change and explore with the family how to overcome barriers to developing the From Fear to Love approach. When deemed appropriate, the Trauma Support Specialist may attend Child and Family Teams (CFT) to provide trauma informed clinical consultation regarding the From Fear to Love approach and to assist with care plan development as well as the implementation, and coordination of services and support that meet the families self-identified needs.

The wraparound process is trauma informed, community-based, culturally relevant, individualized, love-based, and family-centered. The CFT’s work with youth and their families at home, in the community, at school/work setting, and/or virtually/phone due to COVID-19 constraints, to pull the youth and family team together to facilitate the development of the wraparound plan, convene regular CFT meetings, including the youth, their family and their supports, both professional and natural to monitor implementation of the plan and make changes to the plan as needed to promote success for the youth and their family within their community.

Essential Functions:

· Engages and communicates effectively with all agency employees, youth and their families/legal guardians, and all community partners.

· Schedules monthly home visits with families being served. These visits can be virtual when distance prohibits in person visits.

· Reviews and approves Significant Incident Reports, Care Plans, and Transition Plans.

· Ensures the development of individualized care plans for each family using a family centered team-based approach. Provides consultation when needed to assist with completion of these documents.

· Ability to assess and respond effectively to crisis situations. Participates in on-call for youth and families in the wraparound program.

· Advocates for youth and their families when needed by attending MDT’s, court hearings, IEP/SAT meetings, etc.

· Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.

· Utilizes technology provided in order to perform the job efficiently and effectively.

· Documents progress notes and results of visits as appropriate in the family folder in Teams.

· Develops, assists and attends bi-weekly family support group meetings.

· Develops transition plans for families preparing to discharge from the Wraparound Program.

· Meets training requirements as established, including but not limited to, mandated reporting and crisis intervention.

· Other duties as assigned.

Knowledge, Skills, & Abilities

Experience and prior training working with children and adolescents with mental health, substance abuse, and co-existing diagnoses are preferred.

Education & Experience

· Master’s degree in social work or other related discipline. Bachelor's Degree with extensive experience may be considered.

· Experience in wraparound or other clinical services (preferred)

· At least two years’ prior experience working in the social services field

Pay: $75,000.00 - $80,000.00 per year
