The Lakes of Clermont Health and Rehabilitation... is Hiring a HIM / LPN Wound Care Nurse Near Clermont, FL

Overview: The Lakes of Clermont is conveniently located in the southern half of Lake County, within a mile of Orlando Health South Lake Hospital. 40 private and 20 semi-private suites will include individual bathrooms each with it's own shower. Enclosed, interior courtyards with water features will provide multiple locations for relaxing. For entertainment, a movie cinema on-site will provide residents with surround sound theater experiences. Personalized dining services will be provided by our trained culinary staff, including a registered dietitian. Responsibilities:
HIM / Wound Care LPN
In this dual role at The Lakes of Clermont, you will be responsibile for Health Information Management as well as Wound Care in our 80-bed skilled facility.
Responsibilities include:
  • maintaining members' medical records in accordance with facility policies, state and federal regulations
  • establishing a record prior to admission, maintaining a master file for both in-house and discharged members
  • collecting information from the nursing staff, therapists, physicians and other sources to keep records current, monitor and audit records for completeness and accuracy at assigned intervals, in accordance with facility policy, and informs Administrator and Director of Nursing Services of deficiencies in documentation. Wound Care responsibilties include supervising nursing personnel, participating in MDS and care planning for residents with wounds, serve an integral role in the staff training and hiring process, and deliver direct care to all residents.

Qualifications:
  • Must be a Licensed Practical Nurse (LPN) in good standing and currently licensed in our state
  • At least one-year experience in a long-term skilled nursing community
  • Able to relate positively and favorably to customers and families and to work cooperatively with others
  • Demonstrate understanding of state and federal rules and regulations that govern the practices of the facility
  • We are a vaccinated care facility. New hires will need to provide proof of vaccination or other acceptable documents prior to starting employment
