The Judge Group is Hiring a RN Home Health Case Manager Near Everett, WA
***Mileage Reimbursement is provides at IRS rate***
Specific Duties:
1. Hospital discharge calls for High risk, Medicare Advantage panel
2. Panel management of 100 patients within an interdisciplinary team
3. Shared home visits with MD/SW or MA dyad
4. Incoming phone triage
5. In-basket management in a shared pool
6. Visit to patient homes within a 15 minute radius of 3901 Hoyt for assessments.
Must have qualifications/experience:
1. Case management experience
2. Ambulatory Care/outpatient experience
Start & end times of shift:
M-F 8-4:30 or 8:30-5 pm
Day- M-F.
What type of interview process is preferred?
In person, panel
Job Types: Full-time, Contract
Salary: $50.00 - $53.00 per hour
Expected hours: 40 per week
