Works as part of a school-based interdisciplinary team to help students and families with personal and environmental difficulties, which may predispose illness, or interfere with obtaining maximum benefits from medical care. Ensures all adolescents are connected to relevant benefit programs.
This Lead position is expected to assist in training, support, and continuing education of all Health Education and Access Coordinators.

Additional Lead Responsibilities
Assist in the training and continuing education of the Health Education and Access Coordinators Evaluate outreach and enrollment efforts across sites and provide support to all clinic staff in planning future efforts Provide support and best practices along with the School-Based Community RN to Health Education and Access Coordinators in projects such as annual health fairs and student groups Work with each site and School-Based Community RN to identify and conduct targeted outreach opportunities to specific patient groups
o i.e. Yearly CPE outreach, STI outreach, High-Risk/Case-Management Outreach, etc. Work with each site to identify and implement strategies for increased use of SBHC by enrolled patients Work with each site to identify and implement strategies for increasing rates of kept-appointments Provide updates and education on health education topics at monthly Health Education and Access Coordinator and quarterly all-staff meetings Represent the Health Education and Access Coordinator team in meetings with other IFH departments, at trainings and conferences, etc. Work closely with IFH Case Management team to understand updates in benefits and the Marketplace and bring these back to the Health Education and Access Coordinator team Work with all Health Education and Access Coordinators to identify and stock SBHCs with age appropriate, relevant health education materials. Outreach/Enrollment
Conducts student outreach activities to ensure that all students are aware of access to Health Center services. Activities include: classroom presentations, lunch room tabling, overhead announcements, orientation presentations, targeted outreach, etc. Conducts parent outreach activities to ensure that parents/guardians are aware of access to SBHC services for students and general IFH services for other family members. Works to collect enrollment packets for students. Activities include: presenting at parent nights and school community events, producing health center newsletters for families, targeted outreach for missing vaccinations, physicals, etc. Works closely with school administration and staff to ensure that school is aware of services available to students and that this information is communicated to students and families effectively. Activities include presentations at Professional Development events, coordination with school staff in email, robo call, social media, and other communication with parents, etc.
Insurance Access
Individual
Screens children and families for Medicaid, CHP, FHP, FBPB, PCAP and WIC eligibility. Completes the Access NY application for eligible families Provides intensive tracking and follow-up with applicants to ensure that all required supporting documentation is obtained. Works to implement insurance related outreach strategies. Tracks reporting metrics with supervisors on SBHC billing and reimbursement to identify barriers to effective billing and problem-solve/improve SBHC registration workflow.
Classroom and Small Group
Assess individual and community needs for health education on an ongoing basis. Work with Community Health Practicum Adolescent Health Educator to develop/select/adapt and implement a health curriculum to be delivered to students in the classroom and small group setting. Evaluate effectiveness of health education activities and revise as needed with implementation of current best practices.
