Our facility is in need of a full-time RN Clinical Care Coordinator.

JOB SUMMARY: The CCC works with the Nursing Department employees as well as employees in other departments as a team in order to provide the best quality of care to the residents.

 EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:

• Graduate of an accredited school of nursing and current licensure from the State of Georgia as a Licensed Practical Nurse or a Registered Nurse.
• Background or experience in long term care
• Ability to utilize computers
• Supervisor experience of 2-3 years

• Active BLS/CPR certification.
