Job Description:

The licensed therapist will provide direct clinical services to patients, and those involved in their care, who are facing serious illness through consultative visits and telephonic outreach that facilitates advance care planning discussions and offers information/education regarding their illness trajectory and treatment options. Interventions will be provided in a variety of care settings (hospital, long term care facilities, sub-acute care facilities, private residences) and will interface with interdisciplinary health care team members.

Qualifications:

  • Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licenced Professional Councelor (LPC), or Licensed Psychologist (LP) in State of Colorado
  • Willing to drive in personal car around the Denver Metro Area, mileage will be reimbursed
  • Excellent customer service and problem resolution skills
  • Strong skills in verbal and written communication and mediation
  • Ability to work with individuals from diverse backgrounds
  • Comfort with and knowledge of using a personal computer, especially Microsoft Word, Excel, PowerPoint and Zoom
  • Comfort with and knowledge of using a smartphone
  • Experience in palliative care, hospice, serious-illness and/or medical experience strongly preferred

Essential Job Functions:

  • Assist with the completion of advance directives and facilitation of advance care planning
  • Provide information and education regarding illness progression and treatment options in collaboration with their medical providers
  • Support patients, and whomever they define as family, articulate health care goals
  • Advocate for goal-concordant treatment and care for patients
  • Connect patients and families with hospice or other palliative care programs (if appropriate)
  • Assess for and address caregiver concerns
  • Provide information and referrals for emotional/spiritual resources as needed
  • Type and word-process various documents and electronic information
  • Manage, organize, and update relevant data using database applications
  • Communicate effectively and efficiently across all care levels and disciplines
  • Participate in Quality Improvement projects

Job Requirements:

  • Full time, Monday - Friday
  • Maintain all licensure requirements including Medicare enrollment

Job Types: Full-time, Contract

Pay: $71,420.00 - $82,230.00 per year
