As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs.
Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
To be eligible for consideration applicants should have :
As a minimum, an unencumbered State of Illinois R.N. or LPN License, be a graduate of an accredited nursing program and C.
P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. or LPN in a Skilled Nursing Facility setting and (1) year of experience as a C.
N.A. is preferable but we are willing to invest in the right candidate!
