The Grand Rehabilitation and Nursing Center at Pawling , located in Pawling, NY, is seeking LPNs. Postion can be full-time, part-time or per diem.

We have availabilities during all shifts.

LPN Responsibilities :

• Implement and coordinate care delivery in collaboration with physicians and care associates.
• Conduct health and wellness checks for residents regularly.
• Assist in developing and updating individual service plans for residents.
• Maintain open communication with residents, families, and physicians regarding their needs.
• Utilize the nursing process (assessment, planning, implementation, evaluation) for optimal resident care.

LPN Requirements :

• Current and valid NYS LPN License in good standing.
• Ability to work effectively in a team environment.
• Prior experience in a Nursing Home or Hospital preferred but not required
