The Family Resource Center is Hiring an Intake Coordinator Near Berlin, NH

Position Summary: The Intake Coordinator oversees service referrals to the Family Resource Center and schedules and completes the initial intake into services. They gather information about the family and create family records. In collaboration with Program Managers they assign service referrals accordingly. They also answer questions related to services and make referrals when necessary.

Intake Coordinator Responsibilities:

  • Receive and coordinate referrals from physicians, hospitals, facilities, and other community referral sources for the many varied services offered by FRC; including but not limited to Kinship Navigation, parenting education classes, Strength to Succeed, Community events and home visiting
  • Collaborate with a multidisciplinary team to screen incoming referrals to validate program eligibility in accordance with established procedures
  • Collect health insurance information
  • Provide information/referral if a family does not have appropriate insurance coverage
  • Complete all intake documentation and input family demographic information into Theranest and Quickbase
  • Track and monitor all referrals and contact efforts while adhering to program intake timeline
  • Assign cases to the FSS/PP/KN/RS with consultation from various supervisors. Intakes may be completely done or partially done by the intake coordinator before assigning to a staff person.
  • Travel for face-to-face intakes and/or community outreach may be part of the position.
  • Hours will primarily be Monday through Friday between the hours of 8:30 AM – 4:30 PM with some variances/ flexibility as needed to accommodate the families and providers we support.

This position may be a hybrid position with a combination of remote work and in-office time. In-office time will not only include the person’s home office but also FRC satellite offices. Visits with families and referring partners may take place in the office or the community.

MINIMUM REQUIREMENTS:

  • Previous experience providing FRC home visiting/parent education services preferred.
  • Must be friendly in person, on the phone, and when sending faxes, emails, and texts.
  • Must have knowledge of and able to effectively communicate all FRC services, as well as services available to clients in the Coos and Northern Grafton County communities.
  • Maturity, discretion, exemplary interpersonal skills, and sound decision-making necessary.

EDUCATION/LICENSE/CERTIFICATION REUIREMENTS:

  • High school/Ged
  • Current NH State Driver’s License
  • Current vehicle insurance
  • May also require a reliable vehicle for client transport
