Responsibilities

The Service Coordinator will :

• Write concise, accurate and objective Interdisciplinary Notes regarding the current status of the client and related service coordination activities on a monthly basis
• Communicate effectively in person, on the telephone and in writing
• Read, interpret, and follow complex policies, procedures, laws, and regulations
• Interact tactfully and effectively with those contacted in the course of work
• Maintain current workload with accurate and complete documentation
• Conduct meetings, interviews and evaluations; interpret and document data
• Visit clients in their homes, schools, and day programs
• Record accurately and timely on time sheet and mileage forms
• Perform other duties, as assigned

Qualifications :

• Master’s degree in Social Work or related field, and 3 years’ experience working with children (0-3YO)
• Knowledge of Human Growth and Development
• Knowledge of Developmental Disabilities and Case Management functions
• Solid assessment and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
Desired Skills :
• Bilingual

• Experience working with the IDD population
• Specific experience and / or education in one or more of the following areas : Social Work; Rehabilitation; Special Education;

Child Development; *Clinical Counseling; Genetics; and, Developmental Psychology
