Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and criminal recidivism by providing holistic services, housing, and work opportunities to the under-served populations including the formerly homeless and incarcerated, disconnected youth, veterans, and people living with AIDS. Our flagship transitional work program- Ready, Willing & Able- integrates paid work, occupational training, and holistic supportive services to facilitate an individual’s return to mainstream society.

We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 151-unit residence will provide housing for 31 individuals referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 120 units will be apartments for moderate-income households.
The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.
RESPONSIBILITIES:

  • Maintain a caseload and provide supportive case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
  • Monitor and ensure residents are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with completion of reports as needed
  • Other duties as assigned
    QUALIFICATIONS:
    • Bachelor’s Degree in Social Work or relevant field required
    • The ideal candidate must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
    • Excellent case management skills including knowledge of benefits in the NYC Metro area
    • Sensitivity to community issues and needs
    • Outstanding interpersonal and communication skills, both oral and written
    • Strong ethical standards, confidentiality, and personal integrity
    • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)
