Job Summary:This position is primarily responsible for providing case management services and coordination of all services needed by Daily Planet Health Services/Safe Haven consumers, to include medical, mental health, dental, vision, substance abuse, housing, benefits and insurance acquisition, as well as other social service needs. This position will primarily provide services at Daily Planet’s Safe Haven transitional program, serving homeless adults with histories of mental illness and/or substance use disorders, and will focus on assisting participants to successfully transition to permanent housing options. Protects the dignity, privacy and confidentiality of patients and their families; and works collaboratively with other Daily Planet Health Services providers and staff to promote a team-based approach to patient care. The position is also responsible for providing case management services for those patients who qualify for transitional housing services under the Ryan White consortium. Participates in all safety programs which may include assignment to an emergency response team.

Job Requirements:Master’s degree in Social Work required. One (1) to three (3) years of general clinical case management experience required.  Knowledge in chemical dependency, mental health and housing and homeless service systems required. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office. Must demonstrate skill and knowledge in case management, client assessment and engagement and group facilitation.
