Job Summary: This position is primarily responsible for providing case management services and coordination of all services needed by Daily Planet consumers, to include medical, mental health, dental, vision, substance abuse, housing, benefits and insurance acquisition, as well as other social service needs. Protects the dignity, privacy andconfidentiality of patients and their families; and works collaboratively withother Daily Planet providers and staff to promote a team-based approach topatient care.

Job Requirements: Bachelor’s degree in a field ofScience required, Master’s degree in Social Work preferred. One (1) to three (3) years of generalclinical case management experience required. Bilingual (English/Spanish) preferred. Knowledge in chemical dependency, mental health and housing and homelessservice systems required. Skills includeexcellent oral and written communications including telephone, organizational, andpersonal computer including word processing and Microsoft office. Mustdemonstrate skill and knowledge in case management, client assessment andengagement and group exercises.
