Job Responsibilities:
? Prescribes appropriate laboratory and diagnosis procedures related to patients health
status and facilitates appropriate treatment intervention predicated on results of these
procedures.
? Provides comprehensive medical services for members of family, regardless of age or
sex, on continuing basis: Examines patients, using medical instruments and equipment.
? Participates in quality assessment and improvement activities and achieves productivity
goals. Examination of patients, formulation differential diagnostic plans, defines and
orders required diagnostic testing.
? Determines need for consultation and assists in medical care and treatment provided at
the direction of other specialists.
? Orders or executes various tests, analyses, and diagnostic images to provide
information on patients condition.
? Maintains accurate and detailed medical records about each patient, including
observations during examinations (via EMR).
? Writes orders to inoculates and vaccinate patients to immunize patients from
communicable diseases.
? Refers patients to medical specialist for consultant services when necessary for patients
well-being.
? Prepares records and reports as required in support of services rendered in accordance
with established procedures.
? Attend and participate in patient care reports, patient care conferences, team
conferences, professional staff conferences and other appropriate professional activities
only to the extent that such attendance and participation is relative to his/her assigned
cases and/or performance of services as determined by the Medical Director.
? Comply with all laws applicable to and practice within scope of Family Practice, including
HIPPA.

? Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual
orientations and gender identities or expressions.

? Education provides health teaching to patients and families, participates in
development and delivery of educational programs for area health care providers.
Education/Experience Required:
? MD, current active and restricted NYS license
? Current CPR (BLS) certification (Required)
? American Board of Family Medicine (ABFM) (Required)
? DEA Certification
? 1-2 years experience in adult primary care
Other Related Skills/Experience:
? Experience working in community-based health care settings is also preferred
? Experience/An understanding of EMRs is helpful.
? The ability to demonstrate teamwork and effective interpersonal relations skills is
imperative.
? Excellent written/oral communication and computer skills are required.
