Job Type: Full-Time, Day-shift, Monday-Friday

Pay Rate: Starting at $19 hourly depending on experience

Summary: We are recruiting for a dynamic Care Coordinator who can provide consistent quality resident care and service; delivered throughout all shifts. The incumbent is responsible for scheduling organizes and managing all aspects of a patient’s healthcare maintenance and treatment, including accepting management responsibilities of the Nursing Department in absence of the director of resident services.

Requirements

• High school diploma or general education degree (GED)
• Valid LPN License
• Must exhibit superb communication skills.
• Takes initiative in work, seeks out additional information and offers solutions/suggestions.
• Shares on-call responsibilities/shifts as required
