Position: DSP I - Case Manager
Department: Evening Case Management
Position Status: Full Time

JOB SUMMARY

The Case Management Specialist (DSP1) provides needs assessments to clients, and performs duties relating to case management including making home visits, assisting clients in obtaining supportive services, making necessary referrals and providing training regarding independent living skills. May include group facilitating.

ESSENTIAL FUNCTIONS

Essential functions are duties which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation.
The Care Management Specialist performs an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:
  • Assesses needs of client, assists in development of treatment plan, and gathers information relevant to admission.
  • Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs. Assist clients in accessing community resources and provides supportive services as appropriate.
  • Provides group educational services in an effort to improve coping skills and provide a forum in which to share and learn communication skills.
  • Provides written documentation of client activity/treatment plans and periodic written updates in order to maintain compliance with requirement/standards and provide documentation for billing of services. Initiates all correspondence and paperwork related to needs assessments and client service reports (i.e., letters, progress notes, rosters, mailing lists).
  • Arranges after-care services for clients after completion of treatment. Maintains contact with client to follow-up on progress and to ensure client is complying with treatment requirements.
  • Administers drug screening tests, and other screening instruments.

SECONDARY FUNCTIONS

Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.
  • Assists in office tasks when needed.
  • Attends weekly staff and clinical meetings to share in problem solving and addressing specific client issues.
  • Performs other duties as assigned by the Program Coordinator/Director.

COMPETENCIES

  • Demonstrated knowledge of alcoholism and chemical dependency.
  • Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality.
  • Ability in the area of effective communication, interpersonal skills.
  • Ability to complete all documentation in accordance with agency policy.
  • Effective time management/organizational skills.

MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE

  • High School Diploma or equivalent, Behavioral Science Degree preferred.
  • Minimum License Required, Active CDCA
  • Knowledge of alcoholism and other addictions, experience with 12-Step Recovery Program preferred.
  • Valid Ohio Drivers License and ability to maintain insurance coverage preferred.
  • Maintain CPR/First Aid treatment preferred.
  • Maintains applicable agency trainings
