The Coordinating Center is Hiring a Coordinator of Community Services Near Silver Spring, MD

This is a teleworking position, with the expectation that you will be visiting clients in the community setting in the Montgomery County region.

What You Will Do

Our DDA Service Coordinators, who we call Coordinators of Community Services (CCS), are responsible for case management services for people with intellectual/developmental disabilities. The CCS works under Maryland DDA regulations to provide assistance in locating and accessing the full range of services, providers, and other supports for clients which include social, habilitative, vocational, recreational, educational, medical, financial, housing, and legal services. In this role you are at the center of coordination and planning while working collaboratively with your clients, their circle of support, and service providers within the community. Our clients value their partnership with our CCS as they collaborate on the development of person-centered goals, and gain access to the best services and community resources to support their independence in their local community. This position is a home-based, and also requires visits to client homes, day programs, and other service provider locations within Maryland.

As a CCS, you will:

  • Assist participants with intellectual/developmental disabilities in the development of an appropriate and relevant Person-Centered Plan.
  • Explore and access services and supports in a timely manner, ensuring the most integrated setting appropriate to meet the individual’s needs.
  • Advocate on behalf of clients while supporting self-advocacy and participation in service planning.
  • Monitor the provision of services to clients by performing regular information gathering and research relevant to the client's supports and services.
  • Visit clients in settings where supports are provided, or other locations as desired by the clients, at minimum four times during the client's plan year depending on the intensity of coordination needed.
  • Facilitate frequent person-centered planning meetings, including coordinating quarterly visits, annual PCP meetings and interdisciplinary meetings as needed.
  • Conduct required assessments with clients per DDA regulations and program requirements.
  • Assist with all client enrollment-related activities, to ensure continuity of supports and services.
  • Maximize clients’ participation in the team process and promote opportunities for those who choose to lead their team meetings; negotiates and resolves disagreements within the interdisciplinary team.
  • Respond to provider-generated incident reports in a timely manner to ensure the individual’s health and safety is adequately addressed.
  • Consistently and accurately document sufficient level of client services to meet agency billing standards within State of Maryland’s Long-Term Services and Supports (LTSS) system and The Coordinating Center’s internal system.
  • Prioritize, develop, and manage your own schedule to ensure timely completion of work.
