The Connecticut Hospice, Inc. is Hiring a Director of Quality Control- Healthcare BSN Required Near Branford, CT

The Quality Director is a member of the interdisciplinary team who works to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The Quality Director coordinates, organizes, and leads the quality assurance management and process improvement of the Connecticut Hospice. This position is “hands on”. Must support the Connecticut Hospices’ vision, mission, and operating principles. The Quality Director will oversee Infection control and Education related to clinical needs.

Responsibilities Include:

· Responsible for ensuring multidisciplinary quality care and performance improvement activities within CT Hospice are in accordance with established reporting requirements.

· Makes significant contributions to Strategic Plan, QAPI Plan, Compliance Plan, and sets tone for quality operations and performance improvement throughout the organization.

· Performs patient chart reviews, addresses deficiencies in direct and indirect quality care and activities and collaborates with senior leaders and Care Managers.

· Creates and implements company quality standards.

· Accomplishes quality assurance objectives by monitoring, reviewing, and enforcing policies and procedures.

· Trains and motivates employees to ensure that quality standards are met.

· Achieves quality assurance operational objectives by contributing to information analysis.

· Meets financial objectives by estimating requirements, preparing annual budgets, and scheduling expenditures.

· Develops quality assurance plans by conducting hazard analyses and monitoring procedures.

· Validates quality processes by establishing product specifications and quality attributes.

· Assists in the recruiting process.

· Maintains and improves product quality by completing product, company, system, and compliance audits.

· Prepares quality documentation and reports by collecting, analyzing, and summarizing information and trends.

· Submits required data to CMS, Board of Directors, and other stakeholders as needed.

· Communicates effectively and tactfully with patients, recognizing their age, cultural diversity, needs, abilities, personal preferences, and physical condition.

· Demonstrates understanding of nursing process, interpersonal relationships, and dynamics.

· Maintains positive professional demeanor.

· Prepares and oversees site survey’s

· Responsible for the development and implementation of infection control policy and resource to both clinical and non-clinical staff.

· The RN Educator serves as consultant, researcher, instructional and evaluation designer, clinical practice expert, and interprofessional collaborator supporting the clinical development needs of staff throughout the CT Hospice enterprise

Requirements

· Must possess a bachelor’s degree in nursing.

· Must possess current CT RN License in good standing.

· Minimum of two-years-experience in hospice, home care or long-term care.

· Minimum of one year experience in quality control position.

· Maintains current CT driver’s license and auto insurance.

· Must have excellent communication skills and speak in public.

· Must pay attention to details.

· Capacity to maintain accurate records and exercise discretion in handling of confidential information.

· Willingness to work a flexible schedule; including at different sites and programs as needed.

· Ability to maintain assigned work hours.

COMPUTER AND SOFTWARE REQUIREMENTS

· Computer literate and proficient in Microsoft Office Word and Excel, Outlook, plus word processing and presentation software.

· Ability to operate media equipment such as tablets, smartphones, and other electronic equipment.

· Ability to work with general office equipment.

· Ability to work with and understand databases a must and the ability to learn technical skills.
