THE COMMUNITY SUPPORTS NETWORK, INC is Hiring a Client Care Coordinator - HIRING BONUS $1500 Near Kansas, MO
CSN Kansas City is currently offering a $1500 Hiring Bonus for candidates who apply through 7/1/2024 and are successfully hired. The hiring bonus is paid in 3 installments: $200 - First paycheck following completion of initial two-week training, $500 - 3 months of employment and all training complete, $800 - 6 months of employment. *Applicants previously employed by CSN within the past 6 months are not eligible for this bonus.
Job description
As a Care Coordinator, you will develop and lead team members to deliver quality services that help support individuals to learn, grow, and live their best possible lives and thrive as members of their community! This is a salaried position requiring evening and weekend availability.
- Assist with medication management
- Schedule ALL doctor Appointments
- Accompany supported individuals to ALL doctor appointments
- Provide habilitation & documentation
- Ensure all documentation is uploaded into Therap
- Support individuals to practice and maintain good hygiene
- Follow health & safety protocols
- Engage and coach Direct Support Professionals
- Additional duties assigned per company needs
