Position Title : Support Coordinator

On top of this, we are looking for someone who shares our passion for working with adults with disabilities specifically.

Support Coordinator Responsibilities :

• Manage services for individuals with disabilities.
• Conduct intake meetings.
• Develop a plan for services based on the needs of the individual.
• Facilitate the start of services for the individual with state vendors.
• Monitor the individual and the plan on a monthly basis
• On call for emergencies

Required Experience :

• Bachelor’s degree in the field of Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education or Rehabilitation.
• Minimum of one year experience in the field of intellectual / developmental disabilities required
• BOGGS Support Coordination Orientation Certificate preferred but not required
• Must possess valid driver’s license
• Must be able to provide proof of auto insurance.
• Per Executive Order No.283 COVID-19 Vaccination's are required.
