Responsibilities include decentralized coverage of assigned units and teams, coordination of pharmaceutical care, therapeutic drug monitoring, minimization of safety issues, ensuring cost effective pharmacotherapy plans, and medication reconciliation. Collaboration with other healthcare professionals is imperative. We are recruiting a highly motivated pharmacist focusing on both operational and clinical aspects of providing excellent patient care. Other responsibilities include precepting APPE students and PGY1 residents and participation in projects/ research work that supports the department's mission, and representing pharmacy on appropriate committees.Practice Model/Patient PopulationDirect patient care responsibilities for our internal medicine services.There are 9 internal medicine teaching teams.Centralized care responsibilities for all patients through expertise in the medication use process and ensuring timely access to medications.Breakdown of decentralized service coverage and centralized pharmacy coverage: ~30% decentralized / ~70% centralized coverage Pharmacists work every 8th weekend rotating between day and evening coverage.Other scheduling items: 8 weeks of evening shift coverage per year rotating between the decentralized internal medicine position and centralized coverage in the main pharmacy; assigned one holiday per year; a part of the night shift coverage rotation (approximately once every 12-18 months).Responsibilities:Provides safe, efficient, patient-centered, evidence-based medication services.Verifies medication orders and/or dispenses medications accurately according to state and federal regulations.Documents services in a complete, accurate and timely manner.Actively contributes to medication safety initiatives.Utilizes available technology proficiently and to full extent, in their area of practice.Ensures all pharmacy operations are in compliance with national accreditation standards.Maintains professional interactions with patients to facilitate successful patient experiences.Directs staff appropriately to provide safe and efficient care.Maintains professional working relationships with all caregivers.Promotes a positive work environment by providing constructive ideas and feedback to improve services and the department.Educates and trains students, residents, other caregivers, and/or patients.Provides cost-effective care.Accountable for drug use policy adherence.Utilizes time and resources effectively.Other duties as assigned.Education: PharmD or Bachelors of Science degree in Pharmacy from an accredited college of pharmacy.Certifications: Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire.
