Summary
The Case Worker – Clinical Support is responsible for providing case management services.Servicesinclude assistance with education, advocacy, and providing linkages to social services such asscreeningfor eligibility, benefits,andcounselingaround specificneeds.
EssentialFunctions:
Performscase management, including bringingtogether administrative, clinical, and evaluativeactivities, treatment services, community agencies, and other resources needed to assistclients tomeet the goals of their treatment plan. Supports the youth and family in acquiring the necessaryskills and abilities to manage their health, improve family relationships and develop opportunitiesfor prosocial activities and interactions
Supports and advocates for clients in multi-disciplinary meetings, and periodically reassess andevaluates what hasbeen accomplished.
Maintains appropriate professionalclient care documentation including case management andindividual notes in compliancewith all city, state, and federal standards of practice. Provideseducational counseling and advocacy. Conducts home, school, and other visitsas needed.
Meet performance expectations/metrics, asdefined for the position within the assigned program.
Performs other related duties as assigned.
SupervisoryResponsibilities:

MinimumQualifications:
Bachelor’sDegreeinSocialWork,Psychology,orCounseling.
2yearsexperiencein a relatedfield.
Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali, andothers,maybe requireddepending onclient needs.
Goodinterpersonalskills,abilitytoworkwithdiversesocial,economic,andculturalgroups.
ProficiencyinMicrosoftOffice.
Abilitytoplanandcarryoutassignmentsindependently.
Abilitytoprioritize,adheretotimelines,andmulti-task.
QualificationsPreferred:
3yearsofexperienceprovidingrelatedservicestochildrenandfamilies.
ProficiencyinAnasazi,PromisorConnections.
