Responsibilities of the Social Worker include but are not limited to assessing patient and family psychosocial needs and developing plan of care in accordance with course of treatment. Links patient/family to appropriate community resources including information/referral to sources of financial assistance, transportation, food, housing, support groups. Provides short-term counseling to identify problems and facilitate needed referrals. Serves as patient advocate and liaison with physicians, families, and community to ensure continuity of care. Works with financial counselor for financial assistance. Serves as a resource to other staff on community resources and in area of specialty.

REQUIRED SKILLS AND KNOWLEDGE:

• Bachelor’s degree in social work. Additional course work in area of specialty as required.
• Oncology experience preferred
• Licensed with the Texas State Board of Social Worker Examiners
• Knowledge of patient education/training practices and theories.
• Bilingual a plus
• Must be able to communicate with patients, peers, physicians, and general public, and be able to handle multiple tasks.
• Must have general computer skills and ability to learn electronic medical record and computerized scheduling system.
• Must be a team player
