Mid City Community Nursing and Rehabilitation is seeking a Licensed Practical Nurse (LPN) for Audits to join our team!

Qualifications:

• Current Licensed Practical Nurse (LPN) in the State of Louisiana
• Long term care experience required
• Medical facility auditing experience required
• Knowledge of electronic charting system
• Self-starter and the ability to recognize issues and solve them
• Strong computer, medical records, communication and customer service skills
• Great attention to detail

Job Duties:

• Monitor medication administration to ensure that patients are taking their medications accurately
• Report any hazards or concerns to the DON or other appropriate staff member
• Participate in QAPI initiative to improve clinical processes
• Review medical records to identify treatment plans or process errors or problems
• Prepare reports on patient outcomes that are used to improve patient care and nursing practices
• Work with clinical staff to develop, implement, monitor and update quality improvement initiatives
• Develop and implement a system for monitoring the quality of care delivered by nurses and other staff colleagues
• Observe patients to identify any changes in their condition or behavior that may indicate a need for interventions
• Audits throughout the facility will be required
• Ensure all admits and readmit orders are correct
• Confirm all resident appointments are scheduled timely
• Monitor all immunizations, including checking the links system and ensure all immunizations for new admits comply with guidelines
• Make rounds to ensure nursing processes are being carried out effectively
• Ensure IDT is supported with clinical decisions
• Monitor acute outings monthly
• All other tasks assigned by DON
