KEY JOB RESPONSIBILITIES: Provides evidence-based primary and palliative care to assigned patients residing in assisted livings, group homes, private homes and skilled nursing centers. Operates in an empathic manner when caring for patient needs that include diagnostics, treatments and pain control for acute and chronic conditions. Attention to the cultural, religious, and social needs of patients is required with a focus on building strong patient and family relationships to foster goals of care discussions.
Additionally, the Nurse Practitioner:
  • Assesses and examines patients based on Chief Complaint
  • Provides and encourages health promotion and prevention including vaccinations
  • Diagnoses and generates treatment plans based on evidence-based medicine
  • Prescribes and orders diagnostics based on patients needs
  • Completes documentation on all patient interactions within 24 hours of patient visit
  • Educates and counsels patient and family members on the trajectory of chronic, diagnosed diseases
  • Discusses with patient and family, routine, Goals of Care centering on advanced directives
  • Notifies collaborating physician with unusual or unexpected events or problems
  • Communicates intentions and patient interventions to center management when enter and leaving the facility
  • Manages time effectively to ensure patients are seen timely and as needed
  • Adheres to all company medical initiatives and guidelines
  • Maintains current state specific licensure, national certification and DEA
  • Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Follows company outlined corporate compliance guidelines
  • Communicates to the supervisor if unable to meet a patient’s need or customer service issues
  • Participates in the on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Maintain skills and knowledge through certification required continuing education credit hours
  • Organizes work schedule and utilizes time management to be able to attend all required meetings.
  • Complies with agency infection control policies and protocols.
  • Assist with orientation, teaching, and training as requested.
  • The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient’s home (including private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
  • Other duties as assigned by Director.
Professional Experience/Qualifications:
  • Help optimize the capital structure and support strategic initiatives.
  • Customer service orientated with a strong ability to build patient, family and center relationships
  • Hears adequately on the telephone with no more than an amplifier communicates verbally and in writing in English.
  • Maintains the agency’s mission, philosophy, and core values.
  • Adheres to dress code, appearance is neat and clean, wears identification while on duty.
  • Reports to work on time and/or as scheduled. Call offs reported to supervisor
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
  • Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements:
  • Licensed in the state which he/she currently practices.
  • Nationally Certified
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state driver’s license.
  • Proof of current automobile insurance.
  • Current CPR card; Online CPR certification accepted; Hands-on CPR certification preferred.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
Industry experience:
  • Ability to work independently, make accurate, and at times, quick judgments
  • Ability to respond appropriately to crisis outside of a hospital setting
  • Acceptance of and adaptability to different social, racial, cultural and religious modes.
  • Completes all assigned education programs.
  • Active patient contact within past three years preferred.
Education:
  • Graduate with a Master’s Degree from an accredited school of nurse practitioner program
  • Currently licensed as a Registered Nurse and Nurse Practitioner to practice in the state
