The Brookdale Hospital Medical Center is Hiring a Treatment/ In-Take Coordinator Near Brooklyn, NY

Overview:

In collaboration with the Unit Chief the Treatment / In-Take Coordinator is responsible for the operation of the Interfaith Medical Center- Alternative Housing location. The position provides supervision of the day to day activities, and provides treatment supervision to the assigned Mental Health Worker. The Treatment Coordinator oversees the intake process of new residents to the housing program by logging and reviewing referrals, schedule intake interviews. The Treatment Coordinator is responsible for collecting program fees from residents, serves as liaison to Public Assistance, Social Security Administration for residents financial and SNAP benefits.

Responsibilities:
  • Conducts intake interviews for incoming residents and assign admitted candidates to vacant beds.
  • Responsible for coordinating the apartment treatment programs and services.
  • Provides direct supervision to Mental Health Worker and ensure proper coverage of all shifts.
  • Attends a weekly meeting with the Unit Chief and trainings as required.
  • Treatment Coordinator serves as liaison to Public Assistance, Social Security Administration for residents financial and SNAP benefits.
  • Organizes and presents Alternative Housing Program to referral resources in the community such as homeless shelters, club houses, PROS programs.
  • Attends meetings with such organizations as needed.
  • Ensures that the housing program operates day to day in compliance with the regulatory requirements by reviewing apartment safety and environmental check list with staff.
  • Attends case conferences with assigned staff on site or at inpatient facilities.
  • Assists the Unit Chief with providing individual and group supervision of Mental Health workers.
  • Involved in the decisions for disciplinary action, recruitment and hiring of new staff members.
  • Assists in providing educational sessions for the staff including the orientation of new staff.
  • Assists with service plans for residents and may serve as a role model for new staff in delivery of services to housing residents.
  • Assists with in house chart reviews and ensures that all required documents are completed by staff.
  • During the absence of the Unit Chief provides oversight and direction of the clinic programs.
  • Responsible for emergency calls after hours.
  • Performs other duties as required.
Qualifications:

Education:

  • A minimum of an Associate's Degree in Human Services required.
  • Bachelor's Degree preferred.

Experience:

  • One to two years' experience as a Manager of a client based organization.
  • Crisis Prevention Intervention training is desired.

Knowledge and Skills:

  • Basic Computer experience preferred.
  • Experience with Microsoft Office applications to include Outlook, Word, and Excel preferred.
  • Knowledge of congregate care housing, and Office of Mental Health (OMH) regulations a plus.
  • Communicate effectively, write well, and generate electronic data reports.
  • Ability to multi-task, and work as a member of a team as well as being self-sufficient.
  • Detail oriented.

Language:

  • Ability to communicate effectively in English both verbally, and in writing.
  • Excellent interpersonal, organizational, and communication skills a must.

Physical Requirements:

  • Involves standing, walking, sitting, lifting and talking.
  • May include some repetitive motions.
  • Perform other duties as required.
