The Briarwood Community is Hiring a Wellness Nurse - Part Time in Assisted Living Near Worcester, MA

The Briarwood Retirement Community, voted the #1 Continuing Care Retirement Community in Massachusetts by Newsweek Magazine, is looking for a part time (16 hours per week) Wellness Nurse in our Assisted Living community, working from 3:00 pm to 8:00 pm. Working under the direct supervision of the Resident Service Coordinator, the Wellness Nurse will provide a safe, secure and supportive environment for the assisted living residents of Briarwood.
Qualifications:

  • Current LPN license in the state of Massachusetts.
  • Willingness to work with seniors in an environment that upholds their privacy, dignity, choice, independence, and individuality.
  • Excellent communication, organizational and management skills.
  • Demonstrate an overall positive image, enthusiastic attitude, and leadership abilities.
  • Ability to direct, interview and manage personnel.
  • Must have working knowledge of State regulations pertaining to Assisted Living and Special Care Memory Units.
  • Certified Dementia Practitioner preferred.

Essential Functions:

  • Program Development
  • Supervisory Functions
  • Clinical
  • Provide back-up emergency calls for residents when on campus.
  • Provide first aid and emergency services as indicated.
  • Provide evening, holiday and weekend beeper coverage/support.
  • Monitor health status of residents on assessment.
  • Develop individual resident service plans in consultation with resident and family, and facilitates staff follow-through of service plans.
  • Assist Res Service Cood to review and revise service plans in consultation with resident and family at least every 6 months or more often as indicated.
  • Initiate and encourage updating MOLST/POLST for all residents.
  • Provide professional assessment of resident’s need for special services including wheelchair transportation and special dining considerations.
  • Perform assessments on prospective residents for assisted living.
  • Promotes and facilitates interactive problem solving with staff.
  • Coordinates wellness clinic activities.
  • Assist with orientation of new staff.
  • Monitors and maintains records reflecting the resident’s physical and mental condition and reports significant changes in the resident’s condition to his or her physician.
