Responsibilities:

- Familiar with OASIS and Home Health Comprehensive Assessment

- Perform individualized plans of care after the comprehensive assessment

- Provide skilled nursing care to patients in their home
- Administer medications and treatments as prescribed by physicians
- Monitor and record vital signs, such as blood pressure, heart rate, and temperature
- Assess patients' conditions and develop individualized care plans
- Provide wound care and assist with dressing changes when necessary
- Educate patients and their families on proper self-care techniques
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Maintain accurate and up-to-date medical records

Skills:
- Proficiency in ICD-9 coding for accurate documentation and billing purposes
- Experience administering injections and medications
- Strong knowledge of medical terminology and anatomy
- Expertise in acute pain management techniques
- Ability to perform spinal taps for diagnostic purposes
- Excellent assessment skills for monitoring patient conditions
- Knowledge of pain management strategies for effective relief
- Experience in providing care for toddlers and seniors

- Knowledge of AXXESS Electronic Health Records (EHR)
