Education: HS Diploma or GED required

Eligibility/Experience Requirements:

· Valid Driver’s license

· One year’s experience I/DD (preferred)

Job Description:

· Supervise and assist consumers in morning activities, including morning hygiene, clothing selection, dressing for work.

· Perform household maintenance duties as assigned (including such activities as laundry, mopping, dusting/appliance cleaning and similar).

· Provide personal care assistance to consumers throughout overnight shift.

· Ensure the overall safety of the consumers.

· Assist with medication administration.

· Complete all assigned logs and maintain accurate documentation of all occurrences during shift.

· Collaborate on the development/implementation of consumers’ Individual Service plans (ISP). Train and support consumers in all areas of their ISP.

· Train and support consumers in the community including: social, recreational, cultural, religion and education networks.
