THA Concierge Care has one immediate opening for a Weekend On call Hospice RN to support the Hospice care delivery model. In this role, you’ll serve as a professional member of the hospice interdisciplinary team, working together to provide the highest quality care in an atmosphere of compassion and commitment.

Hospice Care Manager primary duties would include:

• Managing all aspects of patient care for a case load of hospice clients in surround areas of Savannah/Hilton Head area.
• Performing admission visits, post-hospital evaluations, routine visits, death visits, and discharges as assigned.
• Supporting continuity of patient care by completing appropriate documentation and ensuring team members are informed of any changes in patient status
• Providing skilled nursing care, education and support to patients and/or their caregivers
• Collaborating with team members to implement, update, and evaluate patient care plans as needed

If you meet the following requirements, this position may be a good match for you:

• Registered Nurse (RN) with current Georgia and SC state licensure.
• One to two years experience in nursing; specific experience in hospice preferred but not required
• CPR certified
• Reliable transportation with proof of insurance, current driver's license and good driving record
• Thorough understanding of patient-centered hospice care and case management principles
• Excellent customer service and communications skills
• Demonstrated ability to function as a contributing member of a multi-faceted team
