We are currently seeking a Scheduler with a can-do attitude and able to perform under pressure. Our ideal candidate will possess excellent communication skills while providing the highest level of customer service to internal and external customers. Scheduler Summary:

Main Responsibilities:

• Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
• Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
