Terros Health is hiring 2 Portable Clinical Care Registered Nurses (RN) to join our medical team in Phoenix, AZ.
Primary Location: McDowell Rd / 48th St
Schedule: Monday-Friday, 5 - 8's
Community Based Medicine
AZ RN License Required
BSN Preferred
NextGen Experience is Preferred
Bilingual in Spanish is a Plus
(Additional Language Differential Pay Available: $2/hour)
The Portable Clinical Care RN position will be responsible for providing comprehensive health care for medically underserved people experiencing unsheltered homelessness through an innovative portable clinical care model. This model integrates street outreach, behavioral health, HIV treatment and prevention services. Performs nursing assessments, vital signs, phlebotomy, diagnostic procedures, assists with medication administration and monitoring, coordinates care with other providers and health care team members.
Duties/Responsibilities Include:
- Provides nursing services. Performs all duties required of an RN in an integrated healthcare system
- Provides education to patients about preventative care, wellness activities and prescribed treatments (Medications for Opioid Use Disorder)
- Performs specialized duties as required by special clinical situations or treatment populations consistent with license and scope of practice, including providing patient engagement in treatment, treatment planning and patient education and support
- Establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate health and treatment goals
- Identifies potential barriers to quality care and opportunities for improvement and reports them to Director of Nursing
- Seeks appropriate Clinical supervision to ensure the provision of effective treatment
- Ensures documentation of patient encounters in accordance with policies, procedures, grant requirements and clinical protocols on the same date of service
- Assists with managing the medication system at the sites, ensuring staff administer and document medication treatment appropriately.
- Works collaboratively with operational and clinical leaders to ensure an efficient workflow
- Supports and participates in huddles, site, medical staff, and agency meetings
- Supports the promotion of the integrated healthcare model
Salary Range (based on 40 hours/week): $70K-$86K Depending on Years of Experience
$$$ STARTING BONUS PLUS 6 MONTH BONUS $$$ TOTAL: $3000*

Qualifications
- Current, unrestricted RN license in the state of Arizona
- BSN degree preferred; ASN degree considered with appropriate experience;
- 3-5 years of equivalent combination of experience and education as an RN working in the hospital or acute care setting, behavioral health or psychiatric experience is preferred. Clinical experience in street outreach and/or HIV prevention and treatment recommended.
- BLS required
- Must pass a Drug Screen and TB Test.
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
- Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions.
- Strong understanding of substance use, addiction, and recovery principles. Knowledge of chronic conditions experienced by those with serious mental illness and / or medications for opioid use disorder preferred.
- Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
- Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role
- Bilingual is preferred
- Will travel in the community, driving the van.
