Responsibilities

Terros Health is hiring an AZ licensed Registered Nurse for a Lead RN - Care Management position for our TIP/Justice Program.

Location: 27th Avenue/ Indian School Rd and new Mesa location: US-60/ Mesa Drive

Schedule: 5- 8's, Monday-Friday

Excellent Salary ($86K-$95K Depending on years of experience)

Correctional Medicine Experience is a Plus

 This position ensures the practice meets quality, cost-effective care to patients, including individuals transitioning from the criminal justice system and those seeking residential or outpatient treatment for substance use disorder (SUD). Interacts with team members and patients as part of the Targeted Investment Program (TIP) clinical team while supervising direct reports. Attends operational meetings and completes TIP grant required reporting. To meet the care needs of these populations, regular travel between health centers, residential treatment center (Maverick House), probation office locations, and jail/prison systems is required. Duties include, but not limited to: 
  • Serves as a member of the TIP team as the Lead RN providing case management support and works collaboratively with clinical leaders and other leadership. Regularly review high risk individuals for appropriateness for nurse case management support, develop individualized patient centered care plans including disenrollment criteria.
  • Ensures the auditing and monitoring requirements are met, specifically for the justice patients and high needs/risk patients; informing leadership of any deficiencies and working together to develop process improvement. Additionally, collaborates with the project management team on the Justice Grant
  • Communicate and coordinate services with the internal and external medical providers. Works collaboratively with all operational and clinical leaders and other leadership to ensure efficient workflows
  • Provides clinical leadership for the Health Centers (Black Canyon and Javelina) – ordering medical and office supplies, management of equipment and staffing of the sites. Maintain working knowledge of the nursing and medical assistant scope of practice.
  • Demonstrates an understanding of the population served and the TIP Justice program requirements. Ensures all patients are treated with dignity and respect. Ensures all reports, logs, reminder calls, and outreach calls are completed daily.
  • Provide nursing care and patient services using a patient centered approach and harm reduction model of care designed to set patients up for success in engaging with primary care and behavioral health services, allowing them to thrive as the reintegrate from the criminal justice system
  • Proficient at utilizing the NextGen and general computer skills to create reports as requested
  • Responsible for efforts to meet program metrics and deliverables. Will work directly as well as supervise efforts and provide direction to team on priorities and tasks working toward meeting measures.
    • Current Program Deliverables (other deliverables may be added):

      • At least annual hemoglobin A1c monitoring for population on medications placing them at risk for diabetes
      • Increasing access to Medication Opioid Use Disorder (MOUD) treatment for individuals reintegrating from the criminal justice system.

  • Participates in program and team meetings as assigned. Promotes collaborative processes across departments to uphold teamwork, excellent internal and external customer service, and contract compliance in collaboration with Program Managers and the Director of Nursing.
  • Maintains working knowledge of MOUD services and operations to facilitate access to care for at risk population. Engaging in collaboration with MOUD Team and MOUD Nurses to facilitate timely access to services for patients at risk and re-entering the community form the criminal justice system or transitioning from residential SUD treatment
  • Supports and participates in huddles and medical staff meetings providing education and guidance related to MOUD and integrated care.
  • In case of patient referral to the Emergency Department or another facility, coordinate care with appropriate staff at the other facility (Mental Health Center, Emergency Department, other Community-Based Substance Abuse and Mental Health Treatment facilities etc).
  • Resolve escalated issues as they arise from operations and requiring coordination with other departments
  • Assist in expanding Terros Health’s external referral network of partner organizations such as hospitals, community health centers, jails, support agencies, and first responders to increase MOUD referrals to Terros Health
  • Educates patients and natural supports to improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and ED utilization

Qualifications

  • Bachelors in Nursing Degree Required (RN); Will consider ADN. (RN)
  • Registered nurse with the Arizona State Board of Nursing – Active and in good standing.

    Justice involved nursing experience is preferred
  • At least 1 years of managerial experience combined with a clinical nursing work history and formal education
  • Excellent written and verbal communication skills with ability to communicate effectively at all levels of the organization.
  • Interpersonal skills which allow for professional and positive relationships with team members, patients, families,

    and other community members. Strong understanding of substance use, addiction, and recovery principles.

    Knowledge of chronic conditions experienced by those with addiction and/or serious mental illness preferred.
  • Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
  • Must have a valid Arizona Fingerprint Clearance card or ability to apply for an Arizona Fingerprint Clearance card (Level 1) within 7 days of assuming role. (If you do not have one when you are hired, HR can assist you with this.)
  • Must pass a Drug Screen and TB Test
