Full-Time Employed: 40 hours/week
Location: 23rd Ave and Dunlap in Phoenix, AZ
Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

The ACT Counselor Develops program activities, facilitates teamwork for outpatient co-occurring disorders services, and conducts individual sessions and classroom-style group treatment services. Works as a team member of an ACT clinical team to assess member needs and goals, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies. Utilizes Motivational Interviewing techniques to effectively engage members into treatment services. Serves as the ACT team’s expert for substance use issues by providing other team staff with education and consultation in respect to substance use issues. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Works under the direct supervision of the ACT Clinical Coordinator.
Actively recruits new clients who could benefit from ACT services
Routinely assesses client need for ACT services, using explicit criteria to determine need to transfer to less intensive services. Gradually and individually transitions eligible clients and ensure continuity of care.Monitors client status during transition and reintroduces services as needed to ensure client stability in the community.
