A MDS LPN is needed at our new and exciting nursing facility in Cleveland, TN.

As a the MDS LPN you will be responsible for coordinating the Minimum Data Set (MDS) assessments and care planning process for our residents in compliance with regulations. This role requires a deep understanding of nursing practices, assessment skills, and regulatory requirements.

ESSENTIAL JOB FUNCTIONS include, but are not limited to, the following. Other duties may be assigned.

Coordinate and complete accurate MDS assessments for residents according to regulations
- Develop comprehensive care plans based on MDS assessments and resident needs
- Ensure timely submission of MDS assessments to meet regulatory deadlines
- Communicate effectively with interdisciplinary team members to ensure coordinated care
- Monitor and update care plans as needed based on changes in residents' conditions
- Participate in quality improvement initiatives related to MDS assessments and care planning

QUALIFICATIONS

- Active Licensed Practical Nurse (LPN) license
- Previous experience working as an MDS Coordinator or in a similar role preferred
- Strong knowledge of MDS assessments and care planning processes
- Understanding of Medicare, Medicaid, and insurance reimbursement guidelines
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Attention to detail and strong organizational skills
