Job Description
Desert Regional Medical Center is a 385-bed acute-care hospital classified as a stroke receiving center and level 2 trauma facility with an innovative, patient centered and evidence-based Rehabilitation Services Department. Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation, joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
GENERAL DUTIES:
The Clinical Pharmacist shall perform clinical, distributive, super visional, and educational functions necessary to support the departmental functions. Furthermore, he/she use critical thinking skills and exercises independent decision-making as needed for the provision of safe and rational medication therapy. In addition, the Pharmacist works to uphold all department related policies and procedures with close attention to medication management duties dictated by the Joint Commission.
DEPARTMENT SPECIFIC DUTIES:
  1. Practices pharmacy in accordance with all applicable regulations and upholds pharmacy policy/procedures as appropriate
  2. Establishes and maintains professional relationships with colleagues, physicians and other care providers.
  3. Helps coordinate department operations and assures responsibility for professional development.
  4. Establishes standards/norms for clinical practice/patient care and evaluates opportunities that improve patient outcomes.
  5. Demonstrates independent leadership and professionalism with respect to the oversight of pharmacy technicians, pharmacy students, and pharmacy interns. Contributes to department research for both pharmacy students and residents
  6. Consistently demonstrates work habits that are in accordance with department standards/norms. Able to troubleshoot problems independently and use the management tree as appropriate.
  7. Is an active participant, who contributes to value of The Pharmacy Team by Helping peers and co-workers to improve services with comments, suggestions, and a positive attitude
  8. Effectively communicates with colleagues, physicians, members of the health care team; and patients when deemed appropriate.
  9. Accurately assesses drug therapy, in relation to patient specific conditions, i.e. compromised renal/hepatic function, hypersensitivity, age-related dose adjustments, department/hospital policies and procedures, therapeutic indication(s), formulary/therapeutic alternatives, administration routes, contraindications, etc.
  10. Recognizes problematic Drug-Drug, Drug-Disease and Drug-Food interactions.
  11. Clinically assesses and appropriately documents report of ADR's and/or medication errors.
  12. Designs and implements an appropriate Pharmacy therapeutic plan to address the identified problem(s).
  13. Monitors for clinical outcomes, and follows up with the patient and health care team.
  14. Effectively documents in the medical record all pertinent drug related interventions.
  15. Maintains compliance with drug related regulation in a hospital setting including but not limited to: USP, State board of Pharmacy, DEA, and CDPH.
  • Maintain knowledge regarding hospital formulary, inventory, and drug shortages
POSITION QUALIFICATIONS:
Required:
  1. Pharm.D degree from accredited university (may consider RPh with extensive work exp.)
  2. Possess a valid CA pharmacist's license (maintain current at all times)
  3. Minimum 6 months' experience in a hospital pharmacy or equivalent clinical practice experience
  4. Current American Heart Association BLS or within 30 days of hire (maintain current at all times)
Preferred:
  1. If routinely staffed in critical care areas will require one or all of the following advanced life support certifications within 90 days of hire or transfer, as applicable (verify with Pharmacy Director):
    1. American Heart Association ACLS (maintain current at all times)
    2. PALS (maintain current at all times)
    3. NRP (maintain current at all times)
    4. S.T.A.B.L.E. (maintain current at all times)
  2. Able to work all shifts and weekends
  3. Excellent written & verbal communication skills
  4. Completion of an ASHP residency program
  5. Certification through the board of pharmaceutical specialties or equivalent
  6. Collaborative practice experience using protocol management skills
  7. Office (Word, Excel, PPT) experience
  8. Familiarity with CPOE, Cerner, or other electronic documentation systems
Pay Range: $65.32 - $104.52 hourly
Individual wages are determined based upon a number of factors including, but not limited to, an individual's qualifications and experience.

Responsibilities
This individual's responsibilities include (but are not limited to): Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records. Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions. Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained. Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions. Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc. Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. Provide education to patients on the use of medications as needed. Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use. Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed. Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.

D. or BS in Pharmacy
Experience:
Preferred: Hospital experience
Certifications:
Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities.
Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
Physical Demands:
Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position.
