Responsibilities Include:

· Assure quality care by adhering to standards set by the physicians.

· Provide in-home care to patients.

· Prepare and record clinical notes for the clinical record.

· Perform assigned tasks ordered, including the administration of treatments and medications, and assessments of the patient’s overall health.

· Follow up with case managers and physicians as needed.

· Complete electronic documentation and paper documentation that meets the standards of state and federal guidelines.

· Participate in continuing education as required.
