Social Work Job Description
Minimum 1 year of experience working in a Skilled Nursing Facility
Temp Social Work is currently seeking an experienced social worker to fill an opening with a skilled nursing facility located in Buffalo, MN. The ideal candidates should be able to perform the duties of a nursing home social worker.
Requirements of the Social Worker
- Bachelor's or Master's degree in Social Work from an accredited school
- Valid Social Work license in the state of Minnesota preferred
- One year of experience working in a nursing home
- Knowledge of community based services - Preferred

Responsibilities of the Certified Social Worker
- Conduct and document psychosocial assessments
- Complete the MDS, care plan and any other required paperwork
- Participate in care conferences
- Facilitate discharge planning
- Communicate with resident families as needed during the course of providing social services and discharge planning
- Facilitate admissions if required

Pay: $28.00 - $33.00 per hour
Expected hours: 40 per week
