Job Title: Social Worker
Location: Greeneville, TN
Duration: 6 -12 Months Contract with possible extension
Responsibilities:
- Provide counseling and support services to individuals and families facing various challenges
- Conduct assessments to determine clients' needs and develop personalized treatment plans
- Collaborate with healthcare professionals, social service agencies, and other stakeholders to coordinate care
- Advocate for clients' rights and access to resources
- Maintain accurate and up-to-date client records and documentation
- Stay informed about community resources and refer clients as needed
