Job Description
GENERAL PURPOSE: Utilizes a variety of treatment approaches to work with primary care patients with associated mental and behavioral health needs. Service goals include, but are not limited to: providing prevention, early intervention and treatment services to patients referred by staff of the Telluride Medical Center (TMC) to improve the functioning and coping skills of individuals and families. The BHC will also be responsible for coordinating patient care by linking patients to other services in the community, monitoring therapeutic clinical outcomes, and developing care plans to assist in accomplishing improved health.
BHC ESSENTIAL DUTIES AND RESPONSIBILITIES
 
  • Delivers and documents individual, group & family therapy, crisis intervention and case management services to patients. These services will be delivered in the Telluride Medical Center
  • Engage in weekly supervision meetings with clinical supervisor to review caseload, development of therapist, therapeutic interventions & modalities, documentation, ethical standards & other relevant topics
  • Conducts mental health status examinations and diagnostic assessment to establish differential diagnosis and treatment plans, with support of in-house clinical supervisor
  • Monitors the administration of the PQH-2 mental health screening tool to all adult patients who visit Primary Care
  • Administers appropriate screening tools as indicated to assess need for mental treatment and services
  • Maintains organized form folders for mental health screening tools and resource information at MA station
  • Delivers patient centered, strengths-based, culturally and linguistically sensitive evaluations
  • Works with patients to identify manageable agreed treatment goals and objectives
  • Under in-house clinical supervision, works towards gaining professional licensure, certifications, and continuing education as required
  • Maintains patient EMR documentation according to the required legal standards
  • Participates in the design of patient services through needs assessment and discussion with PC team members and supervisor
  • Collaborates with other service agencies and therapists within the community to coordinate patient care plans
  • Develop a working knowledge of resources within the community for appropriate referrals
  • Collaborates and consults with PC providers to coordinate patient care plans
  • Works cooperatively and constructively as part of an interdisciplinary team
  • Activate newly diagnosed patients’ readiness to change and family resource support
  • Engages in pre-visit planning to maximize efficiency in warm hand offs and patient encounters
  • Conduct in-services for PC regarding salient mental and behavioral health topics
  • Patient Care
  1. Pre- visit planning for patients in coordination with primary care providers
  2. Document patient care notes in an effective and timely manner
  3. Monitor compliance with care plan and address barriers to patient self-management.
  4. Provide support and resource needs for the patient and family.
  5. Answer general healthcare questions.
  6. Teach patients how to self monitor and track behavior changes
GENERAL RESPONSIBILITIES
  • Responsible for keeping patient information confidential by adhering to Colorado law and Telluride Medical Center policies and procedures.
  • Responsible for compliance with applicable federal, state and local laws; professional practice acts and ethical guidelines; and payor contractual guidelines.
  • Completes required competency training and maintains CME’s as required by the Medical Staff bylaws.
  • Participates as a member of the CPCI Transformation Team.
MISC. DUTIES AS ASSIGNED
  • Supports billing office staff in their efforts to collect patient service fees by documenting appropriately
  • Other duties as assigned

QUALIFICATION REQUIREMENTS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education or Formal Training
  • Master’s level degree in one of the recognized mental health professions
  • Experience treating children, adolescents and families required
  • Experience in an integrated setting preferred
  • Training in conducting mental health status examinations to establish differential diagnosis and treatment plans
  • Colorado State professional license candidate designation required (LPCC or MFTC), with intent to work towards licensure
Knowledge, Skill and Ability: (include materials and equipment directly used)
  • Fluency in English and Spanish preferred
  • Possesses a working knowledge of individual, group, family and crisis treatment approaches and their application to patients with emotional, behavioral, and substance abuse problems
  • Knowledge of and has used the IMPACT model/Collaborative Care
  • Ability to think in terms of population management using approaches such as stepped care and group psychotherapy
  • Knowledge of evidence-based behavioral assessments and interventions relevant to medical conditions
  • A general knowledge of physical health issues of children and adolescents
  • Demonstrates knowledge and application of clinical skills
  • Has ability to produce written documentation and/or electronic records of clinical work in a manner readily understood by other providers and in a manner that meets federal, state, and TMC documentation requirements
  • Ability to summarize service themes from a variety of patient contacts and communicate suggested improvements for service delivery
  • Ability to conduct mental status exams and differential diagnoses based on DSM-V-TR, service planning, assessment case management needs and provide relevant treatment, under supervision of in-house clinical supervisor
  • Ability to assess level of risk and make appropriate clinical judgment for resolution of patient emergencies to effect a safe outcome
  • Advanced Excel, Word, computer and keyboarding skills
  • Knowledge of psychotropic medications and medical information relevant to mental disorders
  • Ability to understand and utilize collaborative documentation when clinically justified and as requested by the patient
  • Ability to interact with people in a professional, friendly manner
  • Ability to receive constructive feedback from clinical supervisor, and incorporate feedback into clinical practice
  • Ability to work independently as well as cooperatively and constructively as part of an interdisciplinary team
  • Ability to think critically and independently
  • Ability to realize and activate potential in every interaction, every day
  • Comfortable practicing in a non-traditional, fast- paced environment outside of the customary 60 minute counseling session
  • Knowledgeable and facile in using Motivational Interviewing, DBT, and Solution-Focused Therapy
  • Ability to use change as an opportunity for innovation and creativity
  • Ability to inspire and model collaborative teamwork
WORKING ENVIRONMENT/PHYSICAL ACTIVITIES:
  • Works extended hours in front of a computer monitor
  • Required to talk and listen
  • Vision abilities required by this job include close vision
  • Be flexible enough to deal with noise, frequent interruptions, and constant changes in scheduling
  • Be able to offer brief, targeted interventions lasting less than 30 minutes
  • Conduct short-term, focused counseling sessions
  • Maintain a small caseload of on-going individual therapy clients, engaging in 50-60 minutes sessions on a weekly basis
OTHER
  • Salary range is between $70,000 - $75,000 per year.
