TeamHealth needs someone like you! We are seeking a part-time NP or PA to work in a skilled nursing facility environment rounding on weekends. This opportunity includes the flexibility to set your own schedule with a round and go practice and no start stop times. You can round in the morning and still have time to enjoy your weekend.

Our locations are in Yale, MI. Yale is known as the Bologna capital of the world. It has a charming downtown with a small town vibe. On the weekends Yale has various events and offers a perfect location to work, play, and enjoy the outdoors.

Key Responsibilities:

• Perform primary care in a skilled nursing facility setting for short term and long-term patients
• Manage chronic disease, diagnose illness and medical conditions, and implement plan of care
• Order and interpret lab results
• Prescribe and monitor medications
• Consult with physicians and coordinate patient care with other healthcare providers
• Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
• Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
• Promote healthy lifestyle changes

Requirements:

• 1 years of experience in a post-acute care setting required
• Valid NP or PA license in the state of Michigan
• Valid DEA w/prescriptive authority in the state of Michigan
