Case Manager - RN, Bethesda, MD

• Hours: Schedule is Monday-Friday 8am-4:30pm, no weekends, off all Federal holidays

Requirements:

• Requires 2 years of strong Acute Care experience in Medical Surgical Case Management experience preferred.
• Requires flexibility, excellent communication skills and computer proficiency
• Demonstrates a high level of resiliency in a fast-paced environment
• Demonstrates excellent critical thinking and collaboration with other healthcare providers
• These contracts are approved for a 52 week length. The candidate will receive extensive training in Case Management.

Responsibilities:

Attends and actively participates in weekly team meetings,Provides and coordinates medical needs for patients. Medication Monitoring and Administration by oral medication or needle injections Orders, reviews, monitors, and discontinues medications as ordered by the physician. Conducts assessments and approves plan of care and service delivery methods. Writes progress notes, summaries, and discharges Implements the treatment and strategies Reports significant/major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazards. Follows patients’ health plans, assists in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures other tasks as assigned.
