Pathways Center’s Outpatient Clinics provide services that include, but are not limited to, individual, group, and family education and counseling.Treatment includes comprehensive, coordinated, and defined services that vary in level of intensity.Outpatient services address a variety of needs such as situational stressors, family relations, interpersonal relationships, mental health issues, life span issues, psychiatric illnesses, addictions, and the needs of victims of abuse and/or trauma.The treating clinicians work jointly with the persons served to maximize their functioning and manage the symptoms of their current situation.The program is designed to enhance the strengths of the individual through recognized treatment interventions.
The responsibilities for the Licensed Counselor position include but are not limited to the following:
·Provide individual and group therapeutic/counseling services to persons diagnosed with mental illness and/or addictive disease
·Arrange appointment schedules at optimal inter
·Conduct face-to-face comprehensive behavioral health assessments to identify appropriate service needs
·Develop initial and ongoing individualized service plans to meet quality of care goals while ensuring compliance with third party payer requirements and regulatory guidelines
·Write timely clinical progress notes that relate directly to the treatment goals and clinical outcomes
·Modify treatment plans based on new developments and needs identified by the individual.
·Provide crisis intervention services, as needed, to existing and walk in individuals as needed
·May perform some case management duties to facilitate and ensure communication, linkage, and continuity of care between individuals, physicians, other organizational components, community resources and service providers.
·Develop and maintain cooperative working relationships with community service providers and other internal services offered by Pathways Center.
