Summary:

The Home Health Nurse, RN is responsible for administering skilled nursing care to patients requiring intermittent professional nursing service.

Registered Nurse Essential Job Functions:
1. Creates positive experiences for internal and external customers that will meet their expectations. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.2. Manages assigned cases and assists office with achieving positive patient outcomes, strategic plan and budget. 3. Conducts a complete assessment of the patient to identify appropriate care needs.4. Document in the electronic medical record and download information daily.5. Completes OASIS at specific point in time, such as admission, resumption of care, recertification, etc. Provides regular supervision to the Licensed Practical Nurses and paraprofessionals who are providing services to the patient according to regulatory guidelines, CMS/Hospice guidelines.6. Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basis following chain of command. Communicates information using current process and technology available to 3HC.7. Ensures that 3HC can bill for services appropriately. 

Requirements:

Registered Nurse currently licensed to practice nursing in the state of North Carolina.Minimum of one-year experience as a professional nurse in an acute care setting.Home Health Experience Preferred. Maintain current CPR certification (BCLS).Vaccination against COVID-19 is mandatory at our organization unless you are approved for an accommodation due to a religious objection or ADA covered medical condition. The organization will also review medical recommendations for a delay in vaccination or for other contraindications to vaccination.
