Division Overview:

Youth & Family Services (YFS) help parents and young people who are striving to become healthy and

restore their relationships. This division works in partnership with human services, child welfare

systems, juvenile courts, and community-based service providers so that together we can help individuals

and families obtain the services and skills they need to live in positive, healthy ways.

We are currently looking for full-time – Teen Pregnancy Prevention (TPP) Group Facilitator

Starting at $41,600 – contingent upon experience, education, etc.

Summary:

This position is responsible for providing direct services to youth and young adults through prevention

and early intervention programming at the Evening Reporting Center and the IYC Chicago Center

utilizing an evidence- based curriculum and health interventions. Programs will address HIV/STD/ teen

pregnancy prevention as well as life skills consisting of adult preparation subjects and wellness.

Essential Duties and Responsibilities:

  • Assist unit supervisor with the implementation of program activities and provide technical guidance to staff, other agencies and service providers as necessary to assure compliance with HIV/AIDS program policy and procedures
  • Performs various technical tasks related to the HIV/AIDS program such as conducting educational presentations, facilitating groups, conducting statistical surveys, developing specific health education plans, and preparing reports and correspondence
  • Facilitate group discussion on Healthy Life Skills. Topics should include but not limited to positive self- esteem and relationship dynamics, friendships, dating, romantic involvement, marriage, communication, decision making, coping, self-management and goal setting
  • Assist unit supervisor in developing and distributing a referral guide inclusive of health (e.g. family planning) and human service resources (e.g. substance abuse and mental health treatment
  • Provide training and staff support. Facilitator will attend and participate in a training of trainer’s event.
  • Facilitator will be expected to train new and existing staff on curriculum and provide technical assistance as needed
  • Prepare, interpret and present reports regarding program operations, evaluation of service and
  • utilization of resources
  • Participate actively in meetings and develop close working relationships with internal team members and external agencies to ensure coordinated, comprehensive client services and to minimize duplication of services
  • Provide referrals for identified needs of the individual and crisis intervention when needed
  • Complete and maintain all required documentation which includes, documenting the results of group activities, reports, progress notes, and other client-related activities including accurate reporting of services 

Requirements

Qualifications:

  • Bachelor’s degree preferred in human services, social work, psychology or related field and/or three years’ lived or professional experience.
  • Knowledge of HIV/AIDS/STD and its impact on minority health issues and community
  • Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders. Specific knowledge necessary for working with special populations.
  • Knowledge of the signs and symptoms of substance abuse including pharmacological factors
  • Knowledge of the ASAM placement criteria and the DSM-IV as it relates to substance-related disorders or able to acquire the knowledge
  • Good computers skills which include proficiency in Microsoft Office applications and the Internet 
  • Good communication, organizational and time management skills
  • Ability to work with a diverse population

The agency currently offers hybrid work schedules that combines in-office and remote work. Employees are required to report to the work site three days per week with the option to work up to two days remotely.
