Tanner Health System is Hiring a Family and Community Coordinator (Days) Near Villa Rica, GA

Information and referrals to patients and families designed to mobilize personal family community resources. Participates in treatment planning for program population as assigned. Responsible for case management of program population and liaison with schools, DJJ officers, medical doctors, family, DFACS, and all other possible referral sources or services. Responsible for providing, under supervision, multi-family education groups to patients and families in the PHP program.
Education
Bachelor's Degree
Experience
One year of related experience. Requires a working knowledge of standard practices and procedures.

Requirements:
Area of Responsibilities
  • COMMUNICATION TEAM WORK CONDUCT: Maintains knowledge of and liaison with community resources for information and referral. Organize and document all family contacts regarding patient progress, program scheduling, family counseling, and family groups. Document patient progress on weekly progress reports and fax one to each community service agent involved with the patient. Communicate vital information gathered about patients to primary therapists, program administrators, and PHP coordinator. Inform PHP coordinator of any discharges or changes in daily attendance each week. Communicates treatment issues and medication concerns to the staff nurse and/or physician daily. Participates in treatment teams with other staff persons and helps write treatment plan reviews in conjunction with treatment team members. Attends staff meetings as needed each week. Establishes and maintains effective working relationships with supervisors, coworkers, and staff of other departments. Performs other duties needed to maintain safe and therapeutic clinical environment. Demonstrates conduct in keeping with Tanner Health Systems ethical standards. Errors may be serious and many involve injury to patient, staff, or self. Works constantly with confidential information and must constantly make decisions concerning true emotional status of patients, i.e., ability to harm self/others. Requires frequent contact with many others to do job. Involves daily interactions with psychiatrist nurses, social services, occupational therapist and patient to provide appropriate services and programs to patient. Will also involve contact with family and outside agencies for patient care on a daily basis.
  • DOCUMENTATION AND MANAGEMENT OF THE MEDICAL RECORD: Schedule and document any and all contacts with physicians, referral contacts, DJJ officers, DFACS caseworkers, and/or school officials for each patient. Schedule and facilitate any needed meetings between program therapists and the above community agencies. Document patient progress on weekly progress reports and fax one to each community service agent involved with the patient. Facilitate and document post-treatment satisfaction surveys and follow-up. Prepares timely documentation for medical record and maintains current case notes. Completes appropriate documentation - treatment plans, progress notes, weekly updates and group notes in specified time limits. Documents calls and related information on designated forms. Participates in discharge planning, referral contacts, verifies follow through, and documentation of discharge plans. Checks and audits charts for documentation and/or charge errors.
  • PARTICIPATION IN THE GENERAL OPERATION OF DEPARTMENT: Responsibly manages resources in a conscientious manner. Assist in maintaining a clean and neat physical environment on the unit, including cleaning kitchens, stocking kitchen with supplies, cleaning and maintaining group rooms, in compliance with infection prevention standards. Seeks and identifies opportunities to improve care, patient flow or utilization of resources. Participates in process improvement initiatives for the department and facility as directed. Participates in the orientation and supervision of new employees and students. Assist in orienting new staff to PHP including rules of PHP and interaction with patients. Assist marketing in promoting for PHP by representing TBHS at school career days and other marketing venues. Participates in training and development activities to enhance own knowledge and skills.
  • PROVISION OF CARE: Assesses psychosocial needs and completes assessment as indicated including recommendations for treatment plan and discharge planning. Schedules admission H&P with program physician and family members. Maintains a waiting list for admissions and contacts family members of waiting list patients weekly. Obtain release of information and contacts all possible agencies persons who currently or previously provided direct services to the patient. Inform them of the patient's presence in the program and gather any relevant information needed for continuity of care. Participates in team planning regarding psychosocial adjustment and continuity of care needs. Contact hospital representatives within 1 working day of patient admission from our program and document contacts daily until patient discharge (obtaining needed information for continuity of care and discharge follow-up). Take attendance for all patients by 4PM each day and make a list of absentees. Parents should be called by 4:30PM to determine whereabouts of patient and conversation documented. Establishes and maintains positive relationships with patients, physicians, allied health professionals, and all referral sources. Works cooperatively with Program Manager in program development.
  • STANDARDS POLICY AND PROCEDURE: Maintains Tanner Medical Center policies on confidentiality. Demonstrates competency in patient care according to the age of the patient and population served. Stays abreast of organization's programs and services. Supports organization's customer service philosophy. Responsible for completion of required facility and departmental education under the direction of the department manager. Complies with other applicable department and/or organization standards and policies. Complies with facility safety, infection control and security program. Supports facility internal and external customer service standards. Attendance of in-service programs as mandated per policy.
Compliance Statement
Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
