Tandym Group is Hiring a Registered Nurse (RN) - Day Habilitation Near Staten Island, NY

A recognized medical services organization in New York City is looking to add a licensed Registered Nurse (RN) to join their growing team on Staten Island.
About the Opportunity:
  • Setting: Residential Group Home - ICF
  • Schedule: Monday to Friday
  • Hours: 8am to 4pm
  • Patient Population: Adults
  • Patient Volume: 14 individuals
Responsibilities:
The Registered Nurse (RN) will:
  • Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unit.
  • Assess and establish health care outcomes for persons receiving services
  • Develop plan of nursing services and oversee the implementation of the healthcare plan by educating person’s receiving services, DSP/Caregivers and families in the plan of nursing services
  • Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs
  • Evaluate the effectiveness of preventive/ongoing healthcare and make recommendations / referrals for alternative care, as necessary
  • Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
  • Complete and maintain all required documentation on the approved electronic health record system
  • Conduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members, as needed.
  • Perform all other duties as assigned
Qualifications:
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Bachelor's Degree in Nursing
  • Experience working with persons with Developmental Disabilities
