Post-Acute Clinical Care Liaison (remote/Lower Bucks - Montgomery County area)

Our Post-Acute Clinical Care Liaison is responsible for the development and expansion of our post-acute management program, establishing relationships and facilitating processes with local hospitals. In this role, you will educate hospital administration, physicians, and discharge planners on the high-quality facilities available for post-acute care. Utilizing Tandigm’s patient lists and EMR alerts, you will proactively identify patients who might benefit from Tandigm programs.
As our Post-Acute Clinical Care Liaison, you will educate patients and families about the benefits of Tandigm’s preferred Skilled Nursing Facility program. You will also work collaboratively with the post-acute team to ensure seamless admission and discharge processes are met. You will identify opportunities to develop and create new programs, building relationships in support of value-based care and aligned with Tandigm’s goals of improving patient care, reducing admissions/readmissions, and providing better transitions of care to support patients and their families.
ESSENTIAL FUNCTIONS
Required Qualifications:
EDUCATION
Bachelor’s degree and/or a professional certification requiring formal education beyond a two-year college
LICENSES/CERTIFICATIONS
Valid U.S. State Driver’s License as well as proof of liability and property damage insurance on any vehicle used for business purposes.
EXPERIENCE
1-3 years’ experience in healthcare or hospital environment or experience as Skilled Nursing Facility liaison
